I have experienced firsthand the pain and life altering affects caused by bad Drs/Hospitals care and am
working hard to educate others, this of course is not to say all Drs and or Hospitals are bad because they aren't but being
proactive is always advisable and could go a long way in saving the lives of you or a loved one.
As such we have forms that are available for you to print free of charge the only thing we ask is that you
aknowledge where you got them, these forms are just basic forms we created in hopes of helping others. We
